Obesity, besides aesthetic problems, is also a very serious risk factor for the onset of medical conditions such as diabetes mellitus, heart disease, but even cancer. According to recent surveys, our country is one of the world’s leading child obesity and overall overweight populations. The last few years have begun to create a nutritional consciousness in Greece and the culture of the gym, healthy diet and diet have just begun to evolve. The science of Bariatrics has helped many overweight patients return to a normal body mass index (B.M.I. ≤ 20). These procedures are not a simple method of slimming, and patient selection should be done with very strict criteria since they are not free of risk, especially since they are addressed to already burdened patients. Surgery such as Gastric Ring Stomach or Stomach Compartmentation have truly impressive results in the loss of accumulated fat, but they cause problems such as “hanging” the skin, creating stretch marks and streaks, and unevenness in fat loss resulting in some fat deposits to be empty only after a loss of too many kilos. Problems following Mass Loss are created for two main reasons. The most important thing is the great speed with which these patients lose weight, and the second is the quality of their skin, which due to the constant tendency it has received has lost its elasticity. As a result, the skin does not shrink and “gather”, the excess remains and new stretch marks are added to those that already exist. The end result can be impressive in terms of weight loss, but the overall picture of the patient does not satisfy him, and despite his over-effort to lose weight he sees the image of his body not being the one he was dreaming about. The same problems can occur even in people who have not undergone such interventions but have lost even a few pounds, with the classic methods such as diet and gymnastics. And here is the bad quality of the skin and the speed at which the pounds are lost. When the desired kilos are lost and the patient reaches or approaches the normal Body Mass Index, the Plastic Surgeon will undertake the necessary complementary interventions to fine-tune the aesthetics. It will take extra cuts of excess skin to treat skin loosening and local liposuction. The stretch marks are treated only indirectly, removing those on the skin that is stretched out and stretching the rest so it is less noticeable.

Abdominoplasty-Gluteus

The most common and necessary in almost all cases of mass weight loss is abdominal pain. It solves the problem of excess skin in the abdomen and buttocks and faces the stretch marks that are between the umbilicus and pubic area due to the fact that this piece of skin is cut off. Especially in men who accumulate fat in the abdomen, the skin’s relaxation is very intense. It is the first of a series of interventions that may need to be done as it restores the trunk on which the rest of the body will be “built”. When weight loss is too great, it is likely that you also need a buttock lift, which is achieved in an operating room along with abdominoplasty, extending the circular section. With the appropriate dermo-lipectomy and strengthening of the abdominal wall, the result is extremely impressive.

Thights

The  surgery solves the problem of relaxation of the inner and anterior surface of the thighs. Excessive skin resection and local liposuction, if needed, solves the problem of relaxation, improves cellulite while the stretch marks are not completely stretched and so there is only improvement in this area since after the removal of skin and fat they are stretched and are less visible. All this is done with the advantage of a hidden incision in the perforated aspect.

Brachioplasty

Hands and arms in particular, often have problems of relaxation and stretch marks. However, the brachinoplasty surgery, as opposed to the Lifting of Feet, is done through a incision which does not hide in any aspect of the body. One should weigh very carefully the pros and cons of such an intervention and move on to it only if the problem of relaxation is too great and unattractive.

Breast

The breasts consist of the mass gland and fat. Overweight patients usually store large amounts of fat in this area, causing men to develop pseudo-gynecomastia (gynecomastia due to the increase in fat only and not in the mass of the gland) and in women giganomastasia, ie excessive increase in breast volume that often causes pain in the back due to weight. In men, if there is still some fat in the breast, the solution is liposuction at one or more stages depending on its amount. The same procedure will be followed in case of skin loosening as it will help the skin stick to its new position limiting the problem. Extrication of excess skin is not a solution for men because the scars that are created are obvious and inconclusive. In women, the range of interventions is greater since the situations we have to deal with are many and different. After massive weight loss, there may be problems of relaxation of the skin of the breasts, stretch marks, fall, excessive volume reduction, almost total loss of fat and regurgitation of the gland, loss of the upper pole of the breast, or even may not be affected especially the breast from the loss of weight and eventually it is disproportionally bulky with respect to the rest of the body. Due to the variety of situations we have to deal with, we can say that any surgery on the breast can be found in patients with weight loss. Simple relaxation and stretch problems can be solved many times, only with the classic breast augmentation method with silicone inserts, while other more complicated incidents will require a vertical cut or inverted “T”. Larger breasts will not have to accept an insert, the classic recovery is enough, while the bigger ones will need to get smaller. The range of breast surgery is great and can be personalized with the collaboration of a plastic surgeon and patient.

New Method: Combination of Abdominoplasty and Breast Augmentation with Limited Scars

Women who lose a lot of pounds usually have trouble relaxing both in the chest and in the abdomen. In these patients, a new, pioneering method of abdominoplasty and recovery, enlargement and restoration of breast relaxation with breast prostheses is applied through a single incision of this abdominoplasty. The innovation lies in the fact that no extra incisions are made for the placement of the breast implants but the access is made by the abdomen and the incision of the abdominoplasty. The relaxation caused by sudden changes in weight in both the hanging and “empty” breasts and the hanging ventricle is treated. The surgery is a combination of abdominoplasty and breast augmentation with silicone. Do not incision in the breasts to place the inserts, but place them as the arrows point on, from the existing incision. The main advantage, apart from the obvious, that the patient in an operation restores two parts of her body (chest, abdomen) is that there are no incisions and therefore scars on the breasts. we have a shift in the center of body weight higher and a redefinition of the relief and its outline, either lost to obesity and weight loss, or having manufacturing problems (small breasts, large circumference)
